Those Online Form Thingys, Anybody Know How To Get Them Done Up In A Quick And Easy Method?

I am doing some marketing surveys on the internet and am thinking of doing up an online form thing so that the website visitors can go to the url and then answer the questions there, and the results are automatically emailed to me. I’ve yahoo-ed and there are some free websites, but they look rather complicated. Anybody know if there are those really idiot-proof user-friendly types where I can use? Thanks.
